All information provided on this application will be verified with the appropriate documentation before your loan can be submitted to underwriting.  By completing this application, you are giving the Bank of Cave City permission to pull your credit report for the purpose of a Mortgage Preapproval. 

The following items will be needed before your loan can be submitted to underwriting: 

Two forms of ID, one of which must be a valid driver’s license or other current photo ID for borrower(s)

Copy of borrower(s) last 3 most recent paystubs. Paystubs must cover most recent 30-day period

Copy of any other form of income, such as retirement, social security, child support, alimony, etc…

Copy of borrower(s) last 2-year tax returns with W2’s and/or 1099’s and other supporting documentation

 If you are self-employed, please provide business tax returns along with current balance sheet and Profit   & Loss statement for business

 Copy of all pages of the last 2 months bank statements for borrower(s)

 Name and phone number of insurance provider
